A patient was referred to our clinic while undergoing incomplete root canal treatment on a premolar after experiencing complications at another dental clinic.

Previously, the dentist had used an ultrasonic tip to assist in enlarging and cleaning the root canal—a modern technique in endodontic treatment. However, improper use and insufficient control caused the canal path to be enlarged in the wrong direction, deviating from the natural anatomical root canal. As a result:

  • It was impossible to reach the necrotic pulp tissue at the root apex, where bacteria are likely to accumulate.
  • The canal could not be completely sealed, leaving voids with a high risk of bacterial reinfection.

✅ Retreatment Under a Dental Microscope: Locating the Correct Root Canal Path Again

At our clinic, we performed retreatment under a dental operating microscope with the following steps:

  • Precisely identified and relocated the correct path of the root canal system using high magnification.
  • Removed all remaining pulp tissue and contaminated materials, thoroughly cleaning the entire root canal system.
  • Filled and sealed the canal with a biocompatible material, helping preserve the natural tooth to the greatest extent possible and preventing recurrent infection.
